This means you have countless ways to make a difference in your role as RN Case Manager by providing quality care.*Schedule: *Monday through Friday, full time days*Why Join the Elara Caring Mission?** Supportive, collaborative environment* Unique, rewarding opportunity caring for patients in their homes* Competitive compensation* Comprehensive onboarding and mentorship* Opportunities for advancement and growth* One-on-One Patient Care* Medical, dental, and vision benefits, 401K and paid-time off for full-time staff.*On an Average day, you will** Ensure that all activities performed align with the vision of Elara Carings board of directors, executive team, and the leadership of the Hospice team.* Assess the assigned case load of hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.* Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.* Assess patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.* Conduct the initial evaluation and re-evaluates the patients nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs.* Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.* Revise the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.* Counsel, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.* Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).* Initiate, supervises, and documents observance of the home health aide and Licensed Practical Nurse/Licensed Vocational Nurse delivery of care per agency policy.* Inform supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues.* Provide appropriate support at time of death and perform bereavement assessment.* Document medication regimen and updates medication profile according to established procedures.* Handle the personal property of patients and their families with care.* Promote Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.* Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.* Performs other duties/projects as assigned.*Qualifications:** Sit, stand, bend, lift and move intermittently and be able to lift 50-100lbs* Driver's License* CPR/BLS* Strong communication skills* Reliable transportation to perform job duties* Bachelors degree required, with additional one year paid professional experience, or one year as a Registered NurseIND123Job Type: Full-timePay: $72,000.00 - $82,000.00 per yearBenefits:* 401(k)* 401(k) matching* Dental insurance* Disability insurance* Flexible spending account* Free parking* Health insurance* Life insurance* Paid sick time* Paid time off* Pet insurance* Referral program* Travel reimbursement* Tuition reimbursement* Vision insuranceMedical specialties:* Hospice & Palliative MedicinePhysical setting:* OutpatientStandard shift:* Day shiftSupplemental pay types:* Bonus opportunitiesWeekly schedule:* Monday to FridayApplication Question(s):* Is your desired wage within the range provided?License/Certification:* RN License (Required)Ability to Relocate:* Roseville, MI: Relocate before starting work (Required)Work Location: On the roadby Jobble
