someone just like you to join our team as a QIDP! Position Overview
The QIDP is responsible for the overall program coordination within
each home assigned. Will develop and implement Individual Plans
(IP’s) that promote self-determination, development, and
independence of those we serve. Ensures all systems and practices
are followed routinely which meet the requirements of all
regulatory agencies. Assists staff in performing all
responsibilities related to the individual’s program services plan.
QIDP is jointly responsible with the Residential Coordinator to
ensure effective supervision of the group home. This position is
also a response for ensuring compliance with federal, state, and
local rules and New Avenues to Independence, Inc. policies and
procedures. Responsible to promptly report to supervisor any
circumstances that negatively affect the health, safety, or
well-being of residents, the agency, or its equipment.
Responsibilities Develops comprehensive I.P. through team process
to meet the individual needs of those individuals served and
monitors active treatment programs and various professional
interventions per I.P. goals and objectives. Coordinates, monitors,
and ensures all aspects of an individual’s I.P. are implemented and
properly documented. Facilitates and participates in the process of
individual admissions, discharges and service substitutions with
their residential sites. Participates in the development and
implementation of the behavior support programs in conjunction with
the behavior support committee. Advocates for individual rights and
needs. Possesses current working knowledge of and complies with all
regulatory compliance agencies. Communicates with all persons
involved in the individual’s life. Leads in MUI process. Maintains
and enters information and eligibility requirements into the DODD
portal; for example LOC, DDP, etc. Assists with QIDP
responsibilities in the absence of QIDP’s as directed by
Residential QIDP Manager. Assumes responsibilities in survey and
inspection preparation with attention to compliance with
expectations on a daily, ongoing basis. Assists with plan of
correction for citations. In coordination with the Core Team (Nurse
and Residential Coordinator) supervises the Direct Support
Professionals regarding I.P., skill building and behavioral
supports program services. Participates in recruiting, orientation,
and training of the Direct Support Professional (DSP). Trains staff
regarding the professional interventions for skill building, active
treatment, behavioral supports and I.P. implementation. Evaluates
staff regarding professional interventions utilized in skill
building, active treatment, behavioral supports and I.P.
implementation. Participates in the evaluation process with the
Core Team. Applies the Right Time Right Place concept with the Core
Team in order to provide adequate presence, support, interaction
and training of staff to address individual needs. Participates in
management responsibilities. Assumes on-call responsibility
according to agency practices and expectations. Performs on-call
duties with the understanding that these duties are a necessary and
important support function. Complies with all Agency’s policies and
procedures (i.e. Policies/Procedure Manual, Employee Handbook, and
etc.) Pursues continuous learning and professional development.
Performs all other duties as required. Requirements: Education
and/or Experience: A Bachelor’s or Graduate level degree from an
accredited college or university; OR hold professional license or
certification issued by the Ohio board of psychology; the state
medical board of Ohio, or the Ohio counselor, social worker, and
marriage and family therapist; OR hold a certificate to practice as
a certified Ohio behavior analyst pursuant to section 4783.04 of
the Revised Code. Certificates, Licenses, Registrations: Must
maintain current certification in Positive Behavior Supports (PBS),
CPR/First Aid, Medication Administration. Must hold valid Ohio
driver’s license and possess a driving record that meets New
Avenues to Independence, Inc. insurance carrier acceptability.
