Encompass Health Corporation, a leading provider in both facility-based and home-based patient care, is seeking a Certified Case Manager to join our dedicated team of healthcare professionals. In this pivotal role, the Case Manager will oversee a diverse patient population, ensuring the implementation of tailored care plans that effectively address individual patient needs. You will collaborate with multidisciplinary teams to optimize patient outcomes, ensuring the highest levels of care coordination and patient advocacy. Develop patient-centered care plans in collaboration with healthcare providers and patients. Coordinate and manage all aspects of patient care across rehabilitation, home health, and hospice settings. Monitor patient progress and adjust care plans as necessary. Serve as the primary point of consultation for patients, families, and healthcare teams on matters pertaining to patient care approach. Ensure compliance with healthcare regulations and standards. Document patient care activities and outcomes for use in continuous quality improvement initiatives. Strong organizational and multitasking skills Excellent communication and interpersonal skills Proficiency in electronic health record (EHR) systems Ability to work collaboratively in a multidisciplinary team Medical Insurance Dental Insurance Vision Insurance Life Insurance Certified Case Manager (CCM) credential is required. Optional: Additional certifications such as Certified Rehabilitation Registered Nurse (CRRN) or Certified Hospice and Palliative Nurse (CHPN) may be beneficial.
